1. Pick up a New Patient Packet at the office or ask the office to email it to you.
2. Drop off the packet and allow for a week to 10 days for the application to be processed by the staff and physician of choice.
3. The office staff will then call to set up a "MANDATORY" New Patient Appointment. This meeting will not be used as an Annual Wellness Exam.
4. You will need to provide a current and active email address as we are a web enabled facility that uses a Patient Portal for 24/7 access.
Tip: It is always best to have a Primary Care Physician BEFORE an emergency as most physician's offices do not accept new patients on an emergency basis.
