1. Pick up a New Patient Packet at the office or ask the office to email it to you.
2. Drop off the packet and allow for a week to 10 days for the application to be processed by the staff and physician of choice.
3. The office staff will then call to set up a New Patient Appointment. This meeting will not be used as an Annual Wellness Exam. You will need to have an appointment in order to be considered an established patient.
4. You will need to provide a current and active email address as we are a web enabled facility that uses a Patient Portal for 24/7 access.
Tip: It is always best to have a Primary Care Physician BEFORE an emergency as most physician's offices do not accept new patients on an emergency basis.
