Every patient over the age of 18 should have an advance directive on file with their primary care provider. For those who do not presently have one on file with this office, we have provided a link to a helpful booklet published by the Colorado Hospital Association. This booklet explains a patient's right to make health care decisions and provides forms necessary under Colorado law to document your choices for medical treatment, including life support, and to appoint substitute decision makers.
We recommend that you print out the advance directive suitable to your situation and bring it in to our office at your earliest convenience.
